• Dropped by HK-8 Aerial Oppression Unit in Operation: Mechagon (Mythic Dungeon).

• The Black Market Auction House (rarely)

Source 3:

• Chance loot from Reins of the Quantum Courser dropped by Chrono-Lord Deios in Dawn of the Infinite (dungeon)

This spider tank drops in Mythic-mode only.
